REQUEST FOR REFUND FORM

Please use this form to request a refund for you child. Please allow 2-3 weeks to receive your refund. If your child has begun practice, please notify the coach also. If you have questions concerning this process, please email the LUYAA Registrar at registrar@luyaa.org

Refund Policy (rev 10/2007)

 All refunds will be directed through the LUYAA webpage, www.luyaa.org.
In the event that Internet access is unavailable, please contact the registrar.
A $10 Administrative fee will be imposed on all refunds requested within 2 weeks of the last registration day at school.
A 50% Refund will be issued for refunds requested before the completion of the second week of practice.
No refunds will be given after the completion of the second week of practice.
Late fees and annual membership dues will not be refunded.

(*) Unless a child does not make a select or travel team and no intramural team is available in the same sport,
or if a child does not make a select or travel team and does not wish to play on an intramural team
or if the
sport is full at the time of payment or canceled for lack of enrollment
